In order to request an act from Mainline Music, please use the following online form- or alternatively please download our DOC file and email it to us Please note that the submission of an offer in no way implies that we agree, or that the artist agrees to the terms, and all confirmed bookings are completed at the point of issuing contracts from our office. Note that all of our artist require a hotel when they perform in addition to any other terms.
